As part of his ascension ritual, Noctis willingly burned away half of his face. The scarred flesh is left uncovered as a permanent reminder of the oath that binds him to his faith. The remaining half is concealed beneath a rectangular iron helmet connected to a network of tubes that feed alchemical compounds directly into his body.
